Job Description:
We're hiring a Certified Occupational Therapy Assistant for a
travel contract role at a Skilled Nursing Facility in Libertyville,
IL. Under the guidance of a licensed Occupational Therapist, you
will help patients regain the skills they need for everyday life.

Minimum Requirements Associate degree in Occupational Therapy from
an accredited college. Active Illinois COTA license required to
start the assignment. BLS certification from AHA or ARC may be
required. Experience working in skilled nursing facility or
long-term care settings preferred. Ability to document treatment
plans and progress notes per facility and state regulations.
Experience with ADL retraining, adaptive equipment recommendations,
and fall prevention programs preferred. Assignment Details Facility
Type: Skilled Nursing Facility. Shift Name: Days. Shift Details:
Days 08:30–16:30. Schedule: Five 8-hour shifts per week. Floating:
Candidate may be required to float to other facilities as needed.
