Job Description Job Description A well-established skilled nursing facility in Illinois is seeking an experienced MDS Coordinator (RN) to oversee the assessment process and ensure accurate, timely completion of all MDS and care planning requirements. The ideal candidate is detail-oriented, highly organized, and experienced with PDPM and Illinois long-term care regulations. Responsibilities Complete and submit MDS assessments accurately and within required timelines Oversee all RAI, PPS, and PDPM processes Collaborate with nursing, rehab, social work, and interdisciplinary teams on care plans Ensure compliance with state, federal, and CMS guidelines Monitor quality indicators, audit documentation, and support survey readiness Educate staff on care planning and documentation requirements Qualifications Active RN license in Illinois Previous MDS Coordinator experience in a skilled nursing facility Strong knowledge of PDPM, CMS regulations, and RAI processes Excellent communication, organization, and assessment skills Ability to work independently and collaboratively with the clinical team What We Offer Competitive compensation Full benefits package Supportive leadership and a stable work environment Opportunity to grow within a respected Illinois long-term care organization IND456
