advancement opportunities What You’ll Do as a Hospice Nurse
Practitioner In this role, you’ll provide expert hospice and
palliative care to patients and families while ensuring comfort,
dignity, and peace during life’s most meaningful journey. You will:
- Perform routine and emergency face-to-face recertification visits
for patients entering their 3rd benefit period or beyond, including
admissions, transfers, and extended stays. - Assess, document, and
communicate clinical findings with the Medical Director, Attending
Physician, and interdisciplinary team (IDG). - Provide physical and
emotional support to patients and families, especially during
crisis situations. - Triage consults and prioritize care based on
patient acuity. - Deliver bedside palliative care and education for
patients and caregivers. - Prevent unnecessary hospitalizations by
meeting patient needs at home. - Maintain accurate and compliant
documentation, billing, and coding per CMS guidelines. - Support
quality care through staff training, continuing education, and
clinical guideline development. - Uphold Elara Caring’s commitment

You Bring to the Team - Master’s Degree in Nursing and national
certification as a Nurse Practitioner - Active state NP license and
ability to maintain licensure - 1 year of clinical nursing
experience (hospice or palliative preferred) - Experience in Family
Medicine, Geriatrics, or Psychiatry is a plus - DEA certification
(if applicable for prescribing in your state) - Valid driver’s
license, insurance, and reliable transportation - Ability to travel
up to 50%, and lift/move up to 50–100 lbs as needed At Elara
