Description:Providence at Home is a growing division of
Providence Life Services, which provides a continuum of healthcare,
focused on senior living. Providence at Home is seeking a RN
Documentation Review Care Coordinator, Full Time in Tinley Park,
IL.The Documentation Review Care Coordinator will report directly
to the Home Health Administrator and will primarily be responsible
for clinical care and record review. This position will work Monday
through Friday; 8AM - 4:30PM at our Corporate Office in Tinley
Park, IL.Summary of responsibilities:Review Start of Care,
Resumption of Care, Recertification and Discharge Oasis completed
by the field staff.Review and evaluate care through services
provided by clinicians, and record review. Instruct and guide
clinicians to promote more effective performance and delivery of
home care services.Review visit notes for clinical staff including
nurses, therapy, and aide notes.Communicates with supervisors and
directors as needed regarding reviews and audits.Complete clinical
record review and discharge chart audits for all other
documentation.Tracks signed face-to-face notes and MD
orders.Participate in meetings and in-servicing as required.All
other duties as assigned.Requirements:Active IL Registered Nurse
(RN) licenseMinimum of two years' experience as an RN, at least one
of which in the area of home health.Preferred experience with Start
of Care, Resumption of Care, Recertification and Discharge Oasis in
Home Health settings.Must be CPR certified.Prolonged or
considerable walking or standing.Able to lift, position, or
transfer patients and supplies and equipment.Considerable reaching,
stooping, bending, kneeling, or crouching.Visual acuity and hearing
to perform required nursing skills.Providence Life Services offers
